Hypnosis Education and Training: In 1997 I was trained in hypnotism in Annandale, VA by the American Institute of Hypnotherapy, the teaching arm of the American Board of Hypnotherapy. I am now an active Certified Hypnotherapist with the National Guild of Hypnotists, and I do annual continuing education to maintain my training at a high level. The National Guild of Hypnotists is the oldest and largest hypnotism organization in the world, and its certification is the most widely recognized credential for the professional practice of the hypnotic arts. As a practicing Hypnotherapist, I am registered with the Washington State Department of Health (Registration Number HP10001384).
As a Hypnotherapist, I am neither a physician nor a licensed health care provider, and may not provide a medical diagnosis, prescribe medications, or recommend discontinuance of medically prescribed treatments. Counselors practicing counseling (including hypnotherapy) for a fee must be registered with the Department of Health for the protection of the public health and safety. Registration of an individual with the Department does not include a recognition of any practice standards, nor necessarily implies the effectiveness of any treatment. You have the right to choose a Hypnotherapist who best suits your needs and purposes; additional services other than my own may be available to you in the community, and you may locate such providers in the telephone book.. You have the right to refuse hypnotism services at any time. As my client, you have a right to be free of physical, verbal or sexual abuse. You also have a right to know the expected duration of sessions, and may assert any right without retaliation.
Redress: As a certified member of the National Guild of Hypnotists, I practice in accordance with its Code of Ethics and Standards of Conduct. Fees: I offer a free, one-time, 1-2 hour consultation service for each new hypnotherapy client. The normal charge for my hypnosis-related services is $60 per hour, pro-rated for portions of an hour in quarter-hour increments, except for the specific techniques listed below. I do not accept credit cards, and prefer payment as services are rendered.

each)

each)

Life Between Lives Regression - $250 per session (usually 1 session, up to 4
hours)
Insurance: I suggest you think of my services as something that you will pay for personally. That will both protect your privacy and help you value the work you are doing more. At this time, insurance companies generally do not like to cover hypnotic services, and I advise you not to expect them to do so.
Confidentiality: I will not release any personal information about the services you receive to anyone without written authorization from you, except as provided for by law. You have the right for access to my written records about you.
My Approach To The Use of Hypnotherapy: Hypnotherapy is one tool that can help to restore balance and wellness in your life in several ways. Hypnotherapy has been shown to be helpful when used under medical referral as an adjunct to medical treatments that address certain physical or emotional conditions. In addition, hypnosis has been shown to be effective in helping clients enhance their personal skills and abilities, regain self-confidence, explore their spirituality, determine and set realistic goals, release apprehensions and concerns, manage situational stress in a positive manner, regain self control, and promote increased wellness in their lives.
Purpose: The purpose of this Disclosure Statement is to comply with the requirements of the Washington Administra-tive Code (WAC), Section 246-810-031 and the Counselor Credentialing Act, Revised Code of Washington (RCW), Chapter 18.19. The purpose of the law regulating counselors (including Hypnotherapists) is:

If the act constitutes a crime, conviction in a criminal proceeding is not a condition precedent to disciplinary action. Upon such a conviction, however, the judgment and sentence is conclusive evidence at the ensuing disciplinary hearing of the guilt of the license holder or applicant of the crime described in the indictment or information, and of the person's violation of the statute on which it is based. For the purposes of this section, conviction includes all instances in which a plea of guilty or nolo contendere is the basis for the conviction and all proceedings in which the sentence has been deferred or suspended. misleading;
d. Incompetence, negligence, or malpractice which
results in injury to a patient or which creates an unreasonable risk that a
patient may be harmed. The use of a nontraditional treatment by itself

in injury to a patient or create
an unreasonable risk that a patient may be harmed;
e. Suspension, revocation, or restriction of the
individual's license to practice any health care profession by competent
authority in any state, federal, or foreign jurisdiction, a certified copy of
the order, stipulation, or agreement being conclusive evidence of the
revocation, suspension, or restriction;
f. The possession, use, prescription for use, or
distribution of controlled substances or legend drugs in any way other than
for legitimate or therapeutic purposes, diversion of controlled substances or
legend drugs, the violation of any drug law, or prescribing controlled
substances for oneself;
g. Violation of any state or federal statute or
administrative rule regulating the profession in question, including any
statute or rule defining or establishing standards of patient care or
professional conduct or practice;
h. Failure to cooperate with the disciplining authority
by:
(1) Not furnishing any papers
or documents;
(2) Not furnishing in writing a
full and complete explanation covering the matter contained in the complaint
filed with the disciplining authority;
(3) Not responding to subpoenas
issued by the disciplining authority, whether or not the recipient of the
subpoena is the accused in the proceeding; or
(4) Not providing reasonable
and timely access for authorized representatives of the disciplining authority
seeking to perform practice reviews at facilities utilized by the license
holder;
i. Failure to comply with an order issued by the
disciplining authority or a stipulation for informal disposition entered into
with the disciplining authority;
j. Aiding or abetting an unlicensed person to practice
when a license is required;
k. Violations of rules established by any health agency;
l. Practice beyond the scope of practice as defined by law or rule;
If you are the victim of unprofessional conduct as defined above, you may inform the
Washington State Department of Health, Health Professions Quality Assurance Division, P.O. Box 47865, Olympia WA 98504-7865 within twenty days after determination is made. You should include the following written information: 1. The name, address, and telephone number of the

2. The name, address and telephone number of the
counselor(s) being reported.
4. A brief description or summary of

Standards of Practice - Energy Healing
All practitioners providing energy healing services at the Evergreen Healing Arts Center are required to agree to the following Standards of Practice:
Standards of Practice
for
Evergreen Healing Arts Center
Energy
Healing Practitioners
PURPOSE
The following Standards of Practice have been developed to guide the
practice of energy-based healing techniques and interventions made available to
the public at the Evergreen Healing Arts Center.
DEFINITION
Energy-based healing techniques and interventions are those that
modify or repattern a client's Physical, Etheric, Emotional, Mental and/or
Spiritual energy fields, and the Energy Centers that control the energy flow to,
from and within the physical body, thus affecting all physical, emotional,
mental and spiritual health and the client's own self-healing processes.
GOAL
The goal of all interventions or therapies provided at Evergreen
Healing Arts Center is to restore harmony and balance in the client's Human
Energy Field, thereby placing the client in a better position to self-heal.
COMPLEMENTARY
All therapies and interventions provided at Evergreen Healing Arts Center
complement traditional health care and are used in collaboration with other
approaches to health and healing.
STANDARDS OF PRACTICE
Standard I
Practitioners at Evergreen Healing Arts Center integrate the therapies
they provide within the scope of their background and training.
This background is clearly represented to the client and to the public in
an appropriate manner such as flyers or brochures that outline one's
credentials, including the successful completion of approved courses of
instruction.
Standard II
A Practitioner at Evergreen Healing Arts Center is a resource within his/her
community to represent the practice of energy-based healing techniques to other
related individual practitioners, professions, institutions and organizations.
Standard III
The Practitioners at Evergreen Healing Arts Center operate from a
theoretical knowledge base of each intervention or therapy they provide, and
they maintain a commitment to continued learning and growing as a healer, well
grounded in the art and in the scientific basis of energetic healing.
Practitioners at Evergreen Healing Arts Center do not diagnose or treat
specific physical diseases, conditions or symptoms, nor do the prescribe
medications. Instead, they obtain
health information and an energetic assessment that serves as the foundation for
understanding the parameters of the current health and healing needs of the
client. Evergreen Healing Arts
Center Practitioners encourage their clients to use the energetic assessments,
information and healing techniques to complement to the services provided by
licensed medical professionals.
Standard V
Practitioners at Evergreen Healing Arts Center assess the client's Energy
System using various tools to determine the current energetic health and healing
needs in the energetic field and centers. This
process is influenced by the Higher Sense Perception ability of the
Practitioner. Various intuitive
gifts determine the information obtained. Information
can include kinesthetic, tactile, visual, auditory, conceptual or knowing cues.
Consequently, the depth and range of data can vary from Practitioner to
Practitioner. However, all
Practitioners use their energetic assessment to guide the selection and
application of appropriate therapeutic techniques and interventions.
Standard VI
Healing is an individually unique lived experience.
Therefore, the Practitioner is a facilitator of the client's own healing
processes, and not the cause of any changes that may occur in the physical body.
Change is within the control of the individual at all times.
The pacing, openness, and intensity of healing techniques is guided by
the client's need at all times.
Standard VII
The techniques and interventions provided by Evergreen Healing Arts Center
Practitioners do not promote a particular religious or spiritual practice, but
respect the individual spiritual beliefs and practices of each client.
The goal is to support the person on his/her own spiritual path using the
energy-based healing techniques as a resource to complement that path.
The techniques and interventions available at Evergreen Healing Arts Center are in accordance with a broad universal concept of spirituality, and an openness to accommodate individual specific differences of opinion about the nature of spirituality. Practitioners come from a wide range of spiritual beliefs, and commit to operate under a universal spiritual basis that respects the spiritual integrity of the individual and that does not impose the Practitioner's beliefs on another.
Standard VIII
Throughout the healing session, the Practitioner gives an explanation
regarding what is going to be done, how and why, based on the energy assessment.
The client is a fully knowledgeable participant in their own healing
process based on his/her abilities.
Due to various presenting situations, such as the client being too sick to receive or take in an explanation, the Practitioner may not give as much information as at other times.
The individual need for an explanation, including depth and scope is to be appropriate for the intervention being provided, and is to be honored. Some want more explanation and some want less. However, all Practitioners are trained and are able to explain to the client what they are doing and why. As the scientific basis for energy-based healing techniques and related healing knowledge is established, the Practitioner is knowledgeable and able to share that information.
Standard IX
The energy-based interventions provided at Evergreen Healing Arts Center are
based on the client's individual needs, and are established from the assessment
of the client's Energy System.
Standard X
After an energy-based intervention is provided, the client's Energy System
is reassessed to determine the effect of the session, the need for further
similar energy-based sessions, and/or the need for referral to other traditional
or complementary health care providers.
Standard XI
A confidential record is maintained on each client and documents the
history, presenting need, assessment, findings, interventions provided,
evaluation, and follow-up or discharge planning.
Standard XII
Referrals are made to other professionals as needed.
Practitioners of energy-based techniques practice within the scope of
their background, training, knowledge and experience.
Standard XIII
Practitioners at Evergreen Healing Arts Center are committed to quality care
and leadership. This requires a
commitment to operate from the Code of Ethics established for Evergreen Healing
Arts Center.
Standard XIV
Practitioners at Evergreen Healing Arts Center receive supervision and
consultation for their practice as needed from their Mentor or other qualified
resources.
